In one of their missions to repay the debt for Isaiah after using the bearer bonds to get cars for Zhou and not getting paid, Slink found a buyer who will give him and Ray tons of cash in exchange for a donor heart that's being transported in a Meat Wagon by the New York City Police.
Ray intercepts the ambulance transporting the heart, steals it and drives to Slink in Chinatown. They both realize that the heart's been recently pulled out of someone and is still pumping, and they have to quickly transport the heart to the buyers at Coney Island before the ice melts and the heart gives out.
Having made back enough money so that Isaiah will never notice that Slink and Ray took his bearer bonds, Ray then gets the idea to use the money to hire some muscle and hit the cocaine shipments heading to the Copacabana, the club that Romero banned Ray and Slink from, from the Latinos. That way, they'll sell the stolen drugs to Zhou, make a lot of money, Ray will hook up with Chen-Chi, and Isaiah's bonds go back in the safe. After hearing about the seemingly fool-proof plan, Slink takes Ray to the Thompsons, a husband and wife team who'll work for Ray and Slink as hired muscle.
This is one of the more difficult missions in the game. The player needs to get to the ambulance being accompanied by a police escort. They have to shoot the driver in order to get the ambulance to stop. Shooting at the cop car is also recommended, as the officer in the car will perform drive-by shooting, unlike the regular cops.

Once the player stops the ambulance, they must steal it and take it to Slink in Chinatown.
The player must now drive to the buyers at Coney Island. One important thing to note is the heart meter on the top left of the screen, acting as a timer, as well as the heartrate beeps in the background. The lower the meter, the more heart beeps will be heard. Drive quickly and carefully, don't crash into traffic or walls, and avoid the cops when possible.

In-game
Ray intercepts the Meat Wagon accompanied by a police escort, kills its driver, steals it, and takes the ambulance to Slink.

Ray and Slink get to the buyer's location at Coney Island before the heart thaws.
Listen, the Copacabana's swimming in coke - some big shipments come in. How about we use the bonds to hire us some muscle, hit the Latinos and take the powder?
